Ymca Membership Agreement

All membership prices are subject to change in writing with 30 days` notice. I understand that it is my responsibility to notify the YMCA of any change of address, bank account information (if the bank transfer is used to pay dues) or credit card information or expiration date (if you use a credit card to pay dues). A one-time membership fee is due for all full members when they join/join Region Y. Membership fees will be waived if a membership is reactivated within 60 days of termination. MEMBERSHIP CONTRIBUTION AGREEMENT Members understand that any changes that affect their monthly draft (i.e., billing information, holdbacks, demotions, termination) require written notice 15 days prior to their next draft date. Any errors must be identified no later than 60 days after the date of publication of the declaration. The YMCA is not responsible for errors that occur more than 90 days after the date of the original error. The YMCA`s draft is an ongoing membership plan. Memberships remain in effect until duly terminated at the YMCA. The YMCA may, in its sole discretion, adjust the monthly membership rate applicable to the membership category.

Members will receive at least 30 days` notice of the change before being charged at the new rate. The Primary Membership Holder is fully responsible for their membership and all persons associated with their membership, including their knowledge of the above guidelines and the waiver of liability. The Member is responsible for providing accurate contact information, including address, phone number, email address and updated billing information, including expiration date and account information. To sign the updated agreement, you will need your barcode, which can be found on your key fob. The YMCA of South Hampton Roads is committed to providing you, your family and guests with a safe environment and programs. The YMCA offers exciting programs that improve lives and include exercise, travel, learning and sport. However, these programs are necessarily associated with a certain risk. The purpose of this Agreement is to obtain your consent to assume these risks on your own behalf and on behalf of any child or other minor for whom you are legally responsible and to ensure that you are aware of and comply with certain YMCA safety and other policies. By clicking “Submit” below, I have given my bank and/or credit card company the authority to account for pre-authorized payments taken in Region Y from my specified account for membership payments as set out above. It is understood that the sending of a pre-authorized payment to the bank or credit card company by the Regional Y when payment is due constitutes valid notice of such payment due on such membership.

If the bank or credit card company redeems the payment by debiting my account, that payment will be my receipt. If a pre-authorized payment is not recognized by that bank or credit card company when it was received by you, it is understood that the payment must be made by me up to that payment. I understand that Region Y, its officers, directors, employees and independent contract agents (Region Y) accept no responsibility for any injury or illness that I or the members of my members may suffer as a result of my physical condition or that may occur on the premises of Region Y before, during, during, during or after participation in or after any activity Y. and hereby release and release Region Y. any claim for injury, illness, death, loss or damage that I may suffer as a result of my participation in these activities. PAYMENT FOR COURSES/PROGRAMS Most courses offered at the YMCA are included in your membership; However, some courses have additional fees that must be paid at the time of registration. I understand that if I wish to cancel or change my membership in any way, I MUST NOTIFY the YMCA IN WRITING FOR 30 DAYS.

(The member must complete the membership termination form in person at the branch). The YMCA conducts regular sex offender screenings on all members, participants and guests. In the case of a sex offender match, the YMCA reserves the right to terminate membership, terminate participation in the program, and remove access to visits. MEMBERSHIP CARDS When joining, members must have their photo taken for identification purposes and receive a membership card. You must present it every time you enter the building to access the facility. Lost membership cards must be reported to the Visitor Centre. The cost of replacing a lost membership card is $5.

Membership cards can only be used by the assigned member; Your membership rights are threatened if you allow others to use your card. You may be asked to present photo IDENTIFICATION at any time to confirm your membership. A photo must be associated with a membership card to be valid. BANK CHEQUE – Monthly contributions are automatically debited from your checking or savings account each month. Outstanding balances must be paid at the beginning of the school year. Written notice fifteen (15) days prior to your next membership project is required for bank changes. I understand that this membership will remain in effect as long as I keep the membership card issued to me. I also understand that membership fees are non-refundable.

Membership cards are the property of the YMCA and must be issued upon request. All records, records, notes, documents and member data are the property of the YMCA of South Hampton Roads. It is my understanding that if I wish to terminate my membership or change my membership in any way, I must give the YMCA 30 days` written notice before my next scheduled draft interrupts my monthly draft. I am encouraged to request and keep a change/cancel receipt for all transactions that affect the payment status or cancellation of the draft on my account. The YMCA also recommends that members check the accuracy of their bank or credit statements on a monthly basis. I understand that the YMCA has the exclusive authority to terminate any membership with or without notice to ensure the safety and convenience of general membership. The YMCA reserves the right to review any application at any time and request additional documentation to ensure accuracy of information and compliance with YMCA of South Hampton Roads membership standards and policies. The YMCA also has the right to request additional documents. The YMCA also reserves the right to review any application for membership and deny membership to any member who could pose a potential threat to the safety of other members. I give the Regional YMCA my permission, without limitation or obligation, to use photographs, videos and/or recordings of which I may include my image or voice or that of others included in my membership for the promotion of the Regional Y, unless I notify the Regional Y in writing before participating in the Y. If my bank or credit card company does not comply with a membership plan for any reason, I am aware that I am responsible for that payment, plus the $15 SERVICE FEE charged by Region Y and any service fees that my bank or credit card company may charge. MEMBERSHIP STATUS Changes to membership status – Changes to your membership, including additions and deletions, as well as changes to the type of membership, must be dealt with in writing in the Visitor Centre.
